mark poulson
The biggest toads I've found are the Stanley Bull Ribbits, and they are only 4.5".
Ring King
It would be mighty expensive but 3:16 Lure Co. has some big frogs. Other than that you might try to contact Wreck'em Baits on Facebook and ask for some of his frogs. They aren't your traditional style frog but we have fished them on big jigheads up here in NorCal for lingcod and done well.

AZ Fisher
http://www.lurecraft.com/Plastic-Mold-880/productinfo/5X880/ Not sure if this is what you're looking for but this should get you close at 5 1/2 inches. Biggest I've found so far.
