bassdog
Here are the hooks you can buy. Barlows also carries the screw in keepers. I'm also interested in how to pour the lead, haven't found a mold for it yet. http://www.barlowstackle.com/acb/showdetl.cfm?&DID=6&Product_ID=1366&CATID=

bassn1
You can make your own mold out of bondo. Just do search for it in the forums or check the knowledge base. I've made several jig molds using this method.
WidowMaker
Do you need a type of jig as shown in the picture or do you simply need a weighted hook? If you need a weighted hook you can make one by wrapping x-amount of lead wire around the shank of the hook and then sealing it with epoxy. If you cant find the proper diameter of lead wire you can pour a long strip of soft lead and use a vice to compress it to the proper thickness. Then use a utility knife and steel ruler to cut the lead into narrow strips.

Mr B
Looks to me like you could use just about any weight forward jig mold that takes a 60 degree hook. Infact it looks very much like my 1/4 arkie head. Take a look at the do-it website at jig molds. Im sure you will find something similar or better. Then just add the keeper to the hook eye. chiefdano
The arkie is the closest. only thing is that arkies are a little wide, and this style is fairly narow. as far as the bondo way, would the bondo fiberglass resin work or would the lead melt it? here's another one, looks like these are all custom made. the first almost looks like a really small swim jig. http://store1.yimg.com/I/yhst-48240192686586_1864_3077135
